Sliding Scale Payments
Please see below the cost structure for sliding scale fees at our clinic. The standard amounts listed are cash pay rates. For yearly income amounts up to and including those listed, you will receive discounts as listed - These numbers are based on federal guidelines.
How do I apply?
Please send an email to payments@unconditionalaccess.org with the following information:
1. Name
2. Date of Birth
3. Any additional contact information
We will write back with a secure link to upload supporting documentation, which can include pay stubs and tax returns. These do not need to be immediately available to receive our sliding scale.
